Item response theory and the dimensions of patient-reported health outcomes

Abstract: The Patient-Reported Outcomes Measurement Information System (PROMIS) has been an initiative of the National Institutes of Health (NIH) that involves collaboration among NIH and researchers at seven university sites. Two of the objectives of the PROMIS Network were to develop and evaluate banks of items measuring dimensions of patient-reported health outcomes, and to create (computerized adaptive) testing systems that permit efficient assessment of those dimensions in clinical trial research. The PROMIS research site at the University of North Carolina at Chapel Hill (UNC-CH) has been engaged in the development of pediatric health outcomes measures. This presentation describes some of what we have learned about the measurement of health outcomes using item response theory (IRT), and some of what we have learned about IRT as we examine health outcomes data, using illustrations from the UNC-CH pediatric measures.
